Required yearly, after May 1st. Good for the entire school year.

Must be signed yearly

2016-2017 LCPS Emergency CARD
This form must be filled out FOR EVERY SEASON. This is the only form required to be turned in every season.

2016-2017 LCPS Student Athlete Handbook
The handbook must be read by each athlete and parent before any participation in any sports.  (Coming Soon)

All Students must have insurance to participate
